Ryzumvi™ Discount Offer Terms and Conditions
Each eligible eye care practice may receive a discount of $200.00* on its first purchase of one carton of 30 single-patient-use vials of Ryzumvi (phentolamine ophthalmic solution) 0.75% (“Ryzumvi”) purchased from an Oyster Point Pharma, Inc., a Viatris company, (“Oyster Point”) authorized distributor (“Ryzumvi Discount Offer”). Authorized distributors can be found at RYZUMVI.com.
To be eligible for this one-time discount, the eye care practice must be a legal corporate entity (e.g., a limited liability corporation, P.C. or limited partnership), not an individual physician, located in the fifty United States or the District of Columbia. Offers are limited at both the healthcare professional (HCP) and practice level. An HCP cannot claim more than one offer. A practice location cannot claim more than one offer.
To receive this discount, the eye care practice must complete the Ryzumvi Discount Offer on-line submission form at RyzumviDiscountOffer.com and submit a photo/scan of the invoice from an authorized distributor showing the Ryzumvi purchase. By submitting the Ryzumvi Discount on-line submission, the practice is confirming that: (i) this is the practice’s initial purchase of Ryzumvi; (ii) the practice purchased Ryzumvi from an authorized distributor; and (iii) the practice has uploaded a photo/scan of the distributor invoice which clearly indicates the Ryzumvi purchase, including the purchase price and that the practice has paid the invoice.
Oyster Point is not responsible for lost, late, damaged, illegible, and/or incomplete submissions. The Ryzumvi Discount Offer cannot be assigned or transferred.
*Valid submission will result in the reimbursement of $200.00 (or the amount actually paid by the practice, whichever is less) on the eye care practice's first Ryzumvi purchase. A reimbursement check will be issued to the practice along with a reconciliation statement. The Ryzumvi Discount Offer is valid for purchases completed by December 31, 2024. Valid submissions must be completed by February 28, 2025. If you do not receive your reimbursement check within 90 days of the electronic submission date on your qualifying submission, please visit ryzumvidiscountoffer.mycheckstatus.com to inquire about your submission status.
Oyster Point reserves the right to rescind, revoke or amend this Ryzumvi Discount Offer without notice and further reserves the right to reject a submission that does not comply with these Terms and Conditions.
